The first step is admitting you need help. Click the button below download the application. Fill it out and email it to [email protected], or mail it to 1616 2nd Ave. N Birmingham AL 35203. After we receive the application, we will need to conduct an interview with the applicant before we can proceed. This can be by phone or in-person.
During the application review process, it is important for the applicant to check in with us (at least by phone) on a regular basis.
